I agree with Seth's info. I also make sure that the patient chart reflects "active involvement" by an on-site provider (physician). We are also sensitive to the fact that many patients will have an office visit co-pay if a 99211 code is used. It is almost better to use a NPP and not have to worry about the level and nature of care being provided. Bob O.
